Nurse Case Manager, Enhanced Step-Down **3,000 Sign-On Bonus**

Pay Rate: $37.45 - $40.01 Hourly
Schedule: Monday – Friday: 8:00 a.m. – 4:00 p.m.

Our focus is supporting individuals by ways of residential housing, mental health and clinical services, and immersing our participants in community engagement. Working closely with the Office for People with Developmental Disabilities and the Office of Mental Health, we support 27 housing locations and 3 administrative buildings.
About the Role:
We are looking for a proactive employee to provide monitoring and oversight of participants’ medical care and ensure their health needs are met while enrolled in the Enhanced Step-Down (ESD) Program for CTI-only services and/or the Transitional Residential Setting (TRS).
Responsibilities:
Monitor the health and well-being of up to 30 participants in the ESD Program, ages 18+
Attend pre-admission, intake, discharge, and annual participant meetings; review medical records, including current medications and treatments
Obtain and coordinate medical services for participants with other agencies, specifically referring partner hospitals
Complete and document participant nursing assessments upon intake and discharge
Develop and monitor Plan of Nursing Services (PONS) for each participant, as needed
Review participant medical records and ensure the maintenance of orderly medical records for each participant, complete pharmacy reviews
Schedule medical appointments, accompany and/or transport participants to medical appointments, advocate for participants, as needed
Review all physician/medical consult forms after medical visits and assure follow up regarding provider recommendations/prescriptions
Stay informed and updated on participant health; complete notes and other documentation required by regulation and Agency policy
Meet with participants referred by partnering hospitals to the ESD program within 24 hours of referral
Follow the CTI case management progression and maintain fidelity with the CTI model
Complete intake assessments to determine needs, goals, and services needed to achieve and maintain stability in the community
Engage individuals in skill building, offer crisis management, collaborate with other service providers, assist individuals in navigating complex systems and collaborate with informal supports in adherence to the CTI model
Provide CTI case management for a small caseload of assigned participants for approximately 9 months
Accompany assigned participants to meetings/intake appointments, as needed
Observe participant progress; adjust supports, make referrals and changes, as needed
Medication Assistive Personnel (AMAP) courses as needed to ensure staff certifications; conduct annual recertification per OPWDD regulations
Observe and review staff competency in medication administration, and make recommendations, as needed
Attend scheduled medical review meetings with ESD Director, CTI & TRS teams
Respond to on-call matters outside of agency hours, as assigned
Provide back up for the Human Resources RN for TB screenings, as needed
Observe confidentiality and safeguards all participant related information
Follow Agency standards and procedures
Maintain compliance with applicable rules and regulations
Ensure compliance with federal, state, and local employment laws and regulations, as well as company policies
Perform Other Duties as Assigned
Education/Experience:
NYS Registered Nurse licensure, required. One year of experience working with individuals with mental health and/or I/DD diagnoses, preferred.
Conditions of Employment:
Must have a valid NYS Driver License in accordance with the Agency Vehicle and Driving policy and transportation necessary to meet program needs. Must respect confidentiality. Must be able to perform duties with repetitive motion. Must be able to lift 75lbs. Must have a flexible schedule and be available for emergencies. Must meet NYS RN recertification requirements and maintain active NYS RN certification.
