Schedule: Monday – Friday: 8:00 a.m. – 4:00 p.m.

Pay Rate: $22.91 Hourly

Our focus is supporting individuals by ways of residential housing, mental health and clinical services, and immersing our participants in community engagement. Working closely with the Office for People with Developmental Disabilities and the Office of Mental Health, we support 27 housing locations and 3 administrative buildings.

About the Role

We are looking for a proactive employee to assist individuals to develop and self-direct some or all of their services outlined in their Self-Direction budget.

Responsibilities include:

• Assist individuals to develop and maintain a planning team/circle of support; provide assistance with scheduling and facilitating planning meetings as needed

• Attend life plan meetings to develop a comprehensive self-directed budget that is consistent with individuals’ life plans; ensure necessary safeguards are included and addressed in the plan; and help identify connections in the community for individuals

• Ensure staff action plans for individuals enrolled in self-hired Community Habilitation and/or Supported Employment (SEMP) are updated and completed twice a year

• Assist individuals to properly document services and to review and submit required documentation to their Fiscal Intermediary in an accurate and timely manner

• Monitor satisfaction of individuals receiving Self-Direction services

• Review expenditure reports provided by the Fiscal Intermediary in conjunction with individuals and their team, and update the budget as needed, so it meets individuals’ needs and remains eligible for Medicaid funding

• Maintain a record with all required self-direction documentation for each individual on assigned caseload who receives support broker services

• Complete and maintain copies of support broker agreements

• Remain actively involved with the local support brokerage learning network

• Recertify support broker status on an annual basis

• Maintain copies of training/certification records

• Facilitate positive communication with other departments and accept assignments in other areas as requested by supervisor

• Conduct self in a professional manner

• Provide person-centered, quality, services

• Follow Agency standards and procedures

• Ensure compliance with federal, state, and local employment laws and regulations, as well as with company policies
• Perform Other Duties as Assigned

Education & Experience

High school diploma/GED, required. Must have completed an OPWDD approved Support Broker training. One year of experience as a Self-Direction Support Broker working with individuals with a disability, preferred.

Conditions of Employment

Must have a valid NYS Driver License in accordance with the Agency Vehicle and Driving policy and transportation necessary to meet program needs.
