About the Role??
We are looking for a proactive employee to use one’s own experience and judgement to carry out the mission of the CTI model and Transitional Residential Services (TRS) for adults (18+), boarding in hospitals and emergency rooms.
Responsibilities include:?
Oversee the ESD Program, including the CTI Team and the TRS Program
Monitor service provision at the TRS site, and with community-based CTI services
Ensure CTI staff, clinical staff, and TRS staff are providing quality and timely services
Ensure all team meetings with ESD/CTI staff, as well as stakeholders, are held regularly and efficiently, according to regulations and best practices
Ensure participants’ timelines are being met according to regulation and evidence-based standards; assist the leadership team to address barriers and support progress
Develop and ensure implementation of an orientation and ongoing training program for CTI staff; clinical staff and assist the TRS Manager in developing and implementing an orientation and ongoing training program for TRS staff
Develop and implement an on-call system to address referrals and emergencies
Implement a system for ongoing supervision meetings and program staff meetings
Provide ongoing direction, support, and regular one on one supervision to staff
Foster a culture of continuous learning and growth; facilitate ongoing training and professional development opportunities for staff members, focusing on person-centered approaches, behavioral management strategies, and trauma-informed care
Complete participant/family/support satisfaction surveys and analyze results
Implement system to measure quality initiatives and program outcomes; monitor the status of program outcomes data and ensure data is submitted in a timely manner for the Agency dashboard
Oversee and monitor all budgets, financial reports, and expenses for the certified residential sites assigned
Ensure that the participants’ rights are protected with regards to their money and personal possessions
Meet with local hospitals to educate them on the CTI process
Participates in meetings with OMH, OPWDD, and all stakeholders and follow up on recommendations made
Provide person-centered, quality, services
Demonstrate effective time management skills
Observe confidentiality and safeguards regarding all participant related information
Follow, and ensure staff follow, Agency standards and procedures
Perform Other Duties as Assigned

Education & Experience??
NYS Licensed Master Social Worker or Licensed Mental Health Counselor certification, required. Licensed Clinical Social Worker or Licensed Mental Health Counselor with diagnostic privilege, preferred. Five years of experience working with people with a mental health diagnosis, and/or intellectual/developmental disability (I/DD) dual diagnosis, preferred. A minimum of three years’ supervisory experience, preferred.
Conditions of Employment?
Must have a valid NYS Driver License in accordance with the Agency Vehicle and Driving policy and transportation necessary to meet program needs.
